Background and aims:
Background and aims: Long-standing type 1 diabetes (T1D) is associated with an increased risk of cognitive decline and
cerebrovascular pathology, commonly attributed to cumulative metabolic burden and cardiovascular disease (CVD). However, a
subset of individuals with prolonged T1D remains largely free from major cardiovascular and renal complications (“escapers”). This
study aims to characterize cognitive function in such individuals and determine whether cognitive performance is preserved relative
to cognitively unimpaired non-diabetic controls.
Materials and methods:
A total of 82 individuals with long-standing T1D from the ESCAPER study in Southern Sweden and 155
cognitively unimpaired non-diabetic controls from the Swedish BioFINDER study underwent standardized, tablet-based
neuropsychological testing with the BioCog test battery. Assessed domains included, memory (word list test and a paired-associate
task), processing speed, and executive function (Trail Making Tests A and B). Domain-specific composite scores were derived. Group
differences were analyzed using linear regression models adjusted for age, sex, and education, with correction for multiple
comparisons.
Results:
One participant with T1D was excluded due to advanced cognitive decline (likely dementia) unrelated to the study condition.
The T1D group had a mean age of 61.4 years (SD 12.3), mean education of 12.8 years (SD 3.2), and 45% were female. The mean diabetes
duration was about 42.6 years (SD 8.3).
The T1D and control groups did not differ significantly in age, sex, or education. No group
differences were observed in composite measures of memory (β=0.06, p=0.50) or executive function/processing speed (β=0.15,
p=0.10).
However, at the individual test level, escapers performed significantly better on the paired-associative memory tasks
compared to controls (β= -0.26, p<0.05), while performing worse on the word list test (β=0.30, p<0.05) and processing speed task
(β=0.30, p<0.001).
Conclusion:
Individuals with long-standing T1D who remain free from major cardiovascular and renal complications exhibit preserved
overall cognitive function despite decades of disease exposure. These findings support the existence of a cognitively resilient T1D
phenotype.
Ongoing multimodal analyses integrating neuroimaging, cardiovascular and metabolic profiling, biomarkers, and genetic
data will help elucidate the mechanisms underlying resilience to cognitive decline in T1D.
